Does anyone’s agency utilize a “Request to Initiate the Dispute Resolution Process Form”?
This form would be for cases involving alleged policy violations. A sworn member may submit a written request using this form for a meeting with the Discipline Resolution Officer to discuss the resolution of the allegations and charges filed against them.
