Agency is looking to update the performance (evaluation) forms currently being used. Seeking the actual forms to document sworn and/or civilian performance evaluations. Welcome any rating standards / directions for objective review to accompany the forms. Thank you.

Good morning, Scott. Attached are the evals used by the City of Lake Mary for both probation and annual evaluations. These are City documents used by all City personnel. I’m unable to upload the excel documents but I’m happy to send them to you via email if you need them; kfries@lakemaryfl.com
