I use Microsoft Word. I have also seen others use PowerPoint. We also have a great organizational chart making program integrated into NEOGOV, if your agency uses that software. An example of the Word version I use is attached.

    Here’s what we say:

    12. Explosives and Caustic/Volatile Liquids
    a. Under no circumstances will the Evidence Specialist/Manager accept any of the following items for storage:
    i. Caustic or volatile liquids,
    ii. Explosives or fireworks,
    iii. Hazardous chemicals,
iv. Known or unknown substances or materials that could be hazardous to the health or well-being of employees.
    b. All such items will be referred to either the ACSO Bomb Squad and/or the City of Gainesville Fire Rescue HAZMAT Team for appropriate disposal.
    c. If any of these items are of an evidentiary nature, they will be documented by video and/or photographed by the submitting employee for evidentiary purposes before disposal.

    The complete policy is attached.

We don’t have a policy specifically on after-action reports, but I can explain how it works. After-action reports are completed any time we need to document/review/assess how something went and we are not using a simple law enforcement report. Most after-action reports are created using what our agency calls an “IOC” (inter-office correspondence) which is the equivalent of a memo. It has a specific format, margins, fonts, etc. . There are a few after-action reports that need to capture very specific information, so those are actually agency forms. Those types of after action reports include Extra Duty After Action Reports, Negotiations Response Team After Action Reports and Hurricane After Action Reports. For the more generalized after-action reports, we typically do them after operational plans, special teams deployments, including Honor Guard, SWAT, Bomb Squad, etc. . Patrol and Communications also do them for various events or something like an evacuation of our Communications Center. I have attached a few examples.

The request to recover costs in any investigation is based on three things: 1.) The rate per hour of the type of employee spent doing the investigation. We charge $50 per hour for a deputy, $25.00 per hour for a Field Service Technician and $30 per hour for a crime scene investigator. 2.) The mileage based on the IRS rate for any transportation. 3.) A $50.00 administrative fee.

Wo do not charge extra for the type of training a particular deputy has. See our attached form.
